SBARs for Ketamine & Midazolam Shortages
Please share this information with your staff in case they receive a box that has been shorted. SBARs attached.
Over the weekend TEMS was made aware that some TEMS pharmacy sites are having issues obtaining a sufficient supply of midazolam 5mg/mL vials. These sites will temporarily decrease the midazolam vial PAR from #3 to a minimum of #2 vials during the supply interruption.
In addition, we have also received emails about intermittent supply issues with ketamine 50mg/mL vials and sodium bicarb syringes again.

As always, the outside of the effective boxes will be marked with any shortage information.
